I believe that the hold is used only when you need to stay in a lower gear to get up a hill or pass someone off a on ramp. It will hold your transmission in third gear I think so that is why the rpms are high and not be driven over 60 mph. The car should be driven with hold OFF 99% of the time. I have three mazdas with this feature and have only used it on accident because it is so close to the shift button. If the transmission shifts fine with the hold OFF, I would say the transmission is normal.
